Summary A change order is the formal documentation of an authorized change in scope to a construction contract. Approval ratifications are presented for the following project(s):

2009 Pavement @ Various Sites Emergency Repair Program (#8446,8450,8454,8458,8462)
Approval #3 $12,405.72
Clairmont Elementary School Emergency Repair Program (#8440)
Approval #3 $70,610.00
Lodi Middle School Water Damage (#8971) Approval # 1 ($28,924.62)

All of the change order items within these Approvals have been authorized in the field by the District, as necessary modifications to complete the project, or to meet District requirements, and are submitted at this time for ratification by the Board of Education. The “A” Exhibits describe the details of the changes for all of the subject projects, while the “B” Exhibits summarize cumulative contract changes. Note that some additional work was authorized on 2009 Pavement @ Various Sites Emergency Repair to fully utilize available state project funding, which at this time is valued at $198,060.00.

Contingency funds were previously budgeted from the following projects:

Recommendation It is recommended that the Board of Education approve ratification of the changes itemized in the attached “A” Exhibits. Exhibits describe the details of the changes for all of the subject projects, while the “B” Exhibits summarize cumulative contract changes.
